Licensed Clinical Neuropsychologist & Clinical Director
Dr. Golzar Kouklan is a clinical psychologist with 10 years of experience in the field. She received her bachelor's degree in psychology from Loyola University and went on to earn her doctorate in clinical psychology from Midwestern University.
Throughout her career, Dr. Kouklan has worked in a variety of settings, including hospitals, federally qualified health centers, and private practice. Her clinical work includes conducting neuropsychological assessments encompassing neurocognitive decline, competency, ADHD, psychodiagnostic, and psychoeducational evaluations.
She has a particular interest in working with individuals struggling with anxiety, depression, and trauma, with extensive training in cognitive-behavioral therapy, acceptance-commitment therapy, and dialectical behavior therapy.
How we work
Step 01
A thorough intake process gathers developmental, medical, psychiatric, and educational history to shape a targeted evaluation plan.
Step 02
A focused, in-person evaluation session using gold-standard neuropsychological measures — paced thoughtfully to the individual.
Step 03
Results are interpreted within the full clinical context, not in isolation. Patterns are analyzed to answer your specific referral question.
Step 04
A detailed written report and a dedicated feedback session ensure findings are clearly understood and actionable for you and your care team.
Rates & payment
Cognitive Space is a private-pay practice. We do not bill insurance directly — this allows us to provide unhurried, individualized care without the constraints of managed care. We provide a detailed superbill you may submit to your insurer for potential out-of-network reimbursement.
Out-of-network reimbursement
Many insurance plans offer out-of-network mental health benefits. Upon request, we provide a superbill — an itemized receipt with the diagnostic and procedure codes your insurer requires. We recommend calling your insurance provider prior to your appointment to ask about your out-of-network psychological benefits and your deductible status.
